Register with us as a New Patient

Any patient wishing to register with the practice has the right to do so if they fall within our catchment area.

The area is as defined as the whole of Kilmarnock and includes Crosshouse, Craigie, Fenwick, Hurlford, Knockentiber, Springside, Gatehead, Moscow, Waterside and Kilmaurs.

Patients who are currently registered with Doctors in the Practice and who reside in Dundonald, Dreghorn, Galston, Cunninghamhead and Symington will continue to be cared for by the Practice until such time as they move/change to another address. Unfortunately we cannot take any new registrations from patients in these areas.

Stewarton patients unfortunately do not reside in an area which we have agreed to offer services.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

Once the registration form has been completed and signed and the proof of identity has been confirmed the Patient is registered with the practice and can make a normal appointment with a clinician (Once their details have been updated on the computer system – this can take up to 5 working days). Please ensure sufficient prescription medication has been obtained from your previous practice before registering with us.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Please download BOTH forms below – the registration form AND the New Patient Health Questionnaire (also listed below) and hand into the practice.  If you have any issues downloading the forms please contact the surgery for further advice. If you wish a family member or friend to be able to discuss your medical care/results etc, please ask for a CONSENT FORM from the practice when you hand in your registration paperwork, as this must be completed in person.
